ABSTRACT

Objectives: The objectives of this study were to evaluate whether infants born to known HIV-positive mothers; but who were not themselves infected with HIV and who were fed a chemically acidified starter formula with prebiotics with or without nucleotides during their first six months; displayed growth rates equal to uninfected infants fed a chemically acidified starter formula without prebiotics or nucleotides. Design: The design was a multi-centre; double-blinded randomised controlled trial. Setting: The study was carried out in four academic hospitals; three in Johannesburg and one in Cape Town; South Africa. Subjects and intervention: The subjects were newborn infants born to consenting HIV-positive women who had previously decided not to breast feed. The infants were randomised to receive one of three milk formulas. The intervention comprised chemically acidified formula without prebiotics or nucleotides; with prebiotics only; or with prebiotics and nucleotides. Outcome measures: The outcome measures were the growth parameters through the first six months of life. Results: Of the 150 randomised infants; 50 did not complete the study and 16 (12.8of those tested) were infected with HIV; leaving 84 infants available for analysis. All three formulas were tolerated well; with no differences in growth parameters seen with the addition of prebiotics and nucleotides. The growth rates of the study infants up to the age of six months were very good; showing an increase in Z-scores from negative values at the time of enrolment in the first week after birth to around zero for length and 0.5 for weight.Conclusions: The three chemically acidified formulas were tolerated well and resulted in good growth over the first six months of life. No benefits were seen with the addition of prebiotics or nucleotides. The growth rates were similar to those found in previous studies of ours on biologically acidified formulas. The chemical acidification of infant formulas appears to be a realistic alternative to biological acidification should an acidified formula be required

"Objectives: To compare the growth of HIV-exposed uninfected infants fed a biologically acidified milk formula with or without probiotics (Bifidobacterium lactis) during the first six months of life; with control infants fed a standard starter formula.Design: Multi-centre; double-blinded randomised controlled trial.Setting: Infants born to HIV-infected women delivering at one of three academic hospitals in Johannesburg; South Africa.Subjects: Consenting HIV-positive women; who had previously decided not to breast-feed; were randomised to receive one of three milk formulas for their newborn infants.Outcome measures: Comparisons of growth parameters through the first four months of life were made between infants fed the acidified formula without probiotics and those fed the control formula (""acidification effect""); and between infants fed the acidified formulas with and without added probiotics (""probiotic effect"").Results: Of 131 randomised infants; 33 (25) did not complete the study and 13 (10) were HIV infected; leaving 85 infants available for analysis. Infants receiving the acidified formula with probiotics had more rapid head growth (p=0.04) and showed a trend towards more rapid weight gain (p=0.06) over the first four months of life than the infants receiving the acidified formula without probiotics.No other significant differences between the feeding groups were demonstrated.Conclusions: Infants in all study groups grew well; with increased head growth and a trend towards increased weight gain for those receiving probiotics.There were no differences in morbidity between the three study groups and no evidence of adverse effects of the study formulas."

ABSTRACT

Variations in somato-genetic patterns in muscle-fibre biology, biochemical metabolic pathways and pulmonary physiology are hypothesized to have been concentrated by natural selection over the centuries in the Afrocentric peoples displaced from West Africa to the New World. These phenotypic and genotypic characteristics are attributed to provide the athletic prowess so well documented in African-Americans. Not the least of coincidence seems to be the influence of the compensatory mechanisms on oxygen transport and its availability to the tissues, in response to the sickle cell gene. The reduced availability coupled with reduced myoglobin in the preponderant fast-twitch muscle fibres which are adapted for rapid energy (ATP) regeneration, all give a NET outcome of muscle anatomical and biochemical advantages which support outstanding performances in athleticism.


Existe la hipótesis de que los patrones somatogenéticos en la biología fibromuscular, las vías meta-bólicas bioquímicas, y la fisiología pulmonar, se han concentrado mediante selección natural a lo largo de siglos, en los pueblos afrocéntricos desplazados desde C1frica Occidental al Nuevo Mundo. A estas características fenotípicas y genotípicas se les atribuye las proezas atléticas, tan bien docu-mentadas en los afroamericanos. Tampoco parece ser coincidencia en lo más mínimo, la influencia de los me-canismos compensatorios de transporte de oxígeno, y su disponibilidad en los tejidos, en respuesta al gene de la célula falciforme. Esta disponibilidad reducida acoplada con la mioglobina reducida en las fibras musculares de contracción rápida preponderantes que están adaptadas para la rápida re-generación de energía (ATP), producen de conjunto un resultado neto en términos de ventajas musculares anatómicas y bioquímicas que constituyen la base de las actuaciones destacadas en el atletismo.

ABSTRACT

O efeito macrofilaricida da infiltração local, com uma alta concentração de cloroquina, dentro da cápsula do nódulo oncocercótico sobre o verme adulto de Onchocerca volvulus foi determinado. Seis semanas depois da infiltração, estudos histopatológicos de nódulos simple demonstraram todos os vermes adultos mortos. Em conglomerados de nódulos a ação da cloroquina foi só sobre os vermes adultos do nódulo infiltrado, não acontecendo a difusão da droga aos nódulos adjacentes. A infiltração de cloroquina a nódulos novos ou de recente formação reduz a carga de vermes adultos dos indivíduos parasitados e pode ser uma alternativa para os altos custos das nodulectomias.


The macrofilaricidal effects of local infiltration of high concentrations of chloroquine into the capsule of onchocercal nodules on adult worms of Onchocerca volvulus was determined. Six weeks post infiltration, histological examination of single nodules showed all adult worms to be dead. With nodule conglomerates, there was localized action of chloroquine only on the adult worms in the infiltrated nodule, with no diffusion of the drug to adjacent nodules. Chloroquine infiltration of young, recently formed nodules to reduce the adult worm load of infected individuals may be an alternative method to costly nodulectomy.

ABSTRACT

DNA extracted from peripheral blood of two Ecuadorian patients showing severe digestive pathology was amplified by the polymerase chain reaction using a Trypanosoma cruzi specific oligonucleotide primers derived from the primary sequence of a cDNA encoding for a 24 kDa excretory/secretory protein. The positive PCR results together with the clinical findings confirmed that both patients had a digestive pathology due to Chagas' disease. This pathology could be more frequent than previously described in the chagasic endemic regions of Andean countries.


DNA obtido do sangue periférico de dois pacientes equatorianos, que apresentavam severa patologia digestiva, foi amplificado pela "polymerase chain reaction" (PCR) utilizando os oligonucleotídoes específicos do Trypanosoma cruzi, derivados de uma seqüência primária de cDNA codificado de 24 kDa proteína excretória/secretória. Os resultados positivos da PCR junto com os achados clínicos confirmam que os dois pacientes tinham uma patologia digestiva de origem chagásica. Esta patologia poderia ser mais freqüente que a descrita previamente nas regiões endêmicas chagásicas das cidades dos Andes.
